Community Concern Reporting Form
Help us support our unhoused neighbors by reporting concerning camp incidents.

What is the purpose of this form?
OlyMAP uses this community concern form to track and understand concerns brought up by camp residents as well as from the general public.

Who can use this form?
While the concern submission form is primarily intended as a way for camp residents to share information related to concerning incidents with us safely and anonymously, any community member can submit a concern related to incidents at communities served by this program through this form.

What are the limits to the response OlyMAP can address?
OlyMAP’s ability to respond to a concern is limited to providing harm-reductive, trauma-informed support to those affected by an incident described in the concern. This support may include, but is not necessarily limited to, providing survival or hygiene supplies, connection to other resources or services, mediation, or advocacy on behalf of affected parties. OlyMAP has no site control over camp communities or authority over camp residents and is not responsible for the actions of camp residents. OlyMAP has no ability to take enforcement action or conduct rigorous investigations in response to criminal infractions. For concerns about criminal activity, please additionally make a report to 911.

What follow-up is available?
OlyMAP will respond by phone, text, or email within one week of receiving a report if requested by the reporter letting them know the report has been received and investigated. When reports contain information about a crime or potential crime, follow-up may be delayed until the conclusion of a criminal investigation by the police. If information gained through investigation involves participant names or demographic information, follow-up will be limited to general information.
